Under the existing healthcare model, there is no one to organize the care and the patient is frequently sent to multiple specialists where sometimes they can receive conflicting information. The patient is asked to make difficult decisions during a time of confusion and stress. Case management is meant to assist the patient in obtaining quality care. Acting as a patient advocate, the case manager's role is to help the patient understand the disease and various treatment options.
Case management is a process that assesses, plans, implements, coordinates, monitors and evaluates options and services to meet the individual's health needs. Case management results in quality cost-effective outcomes. The case manager is an advocate for the patient while still focusing on GVHP coverage specifications, creating a win-win situation for the patient, healthcare team and GVHP.
GVHP's certified case managers can work with members faced with major health obstacles to help them achieve their treatment goals. The case managers are available exclusively for GVHP members. GVHP's case managers go directly into the hospitals to meet the patient and facilitate their care. This hands-on management ensures individualized personal care that promotes positive healthcare outcomes.
GVHP Case Managers are part of your extended GVHP Family Health Center team. As a GVHP Member your Primary Care Practitioner will coordinate your care to include Case Management services.
