At your first Health Center visit you will be asked to complete a Health Risk Appraisal (HRA). This form asks questions about your health and personal health habits. It also asks for information about your immediate family. Your Health Center practitioner reviews the questionnaire and looks for things that might be considered "high risk."

As part of your continuing relationship with GVHP, you will be asked to periodically complete a new Health Risk Appraisal. More information can be found in the Member Handbook section regarding Preventive Health and Wellness.
