Provider Directory - Urgent Care

 

The Urgent Care Center is available for Grand Valley Health Plan members for after hours care. Urgent care is when a member has an illness or injury that causes a degree of discomfort or disability that requires same day attention. Minor conditions such as abdominal pain and minor lacerations can be treated at the Urgent Care Center. If you have an emergency, an immediate threat to life or limb, we recommend you go to the nearest Emergency Room.

Regular Office Hours:
Monday through Friday: 6:00pm - 9:30pm
Saturday and Sunday: 10:30am - 5:30pm
Holiday Weekends: 10:30am - 2:30pm

Physician Assistant: Michael Moore, B.S.B.A., P.A.-C.
  • Certified, National Commission on Certification of Physician Assistants.
  • Mr. Moore received his undergraduate degree from Aquinas College and is a graduate of the Physician Assistant program at Cook County Hospital in Chicago, IL.

Physician Assistant: Stephen Duchemin, B.S., P.A.-C.
  • Certified, National Commission on Certification of Physician Assistants.
  • Mr. Duchemin received his undergraduate degree from the University of Massachusetts and is a graduate of the Physician Assistant program at Northeastern University.

Family Nurse Practitioner: Renee Hodges Bell, C.F.N.P., R.N.
  • Ms. Bell received her undergraduate degree from Michigan State University in 1989 and her MSN from Old Dominion University in Norfolk, VA in 1996.
  • She served in the US Navy as a nurse corps officer from 1989-1991. Her speciality is in the diagnosis and treatment of acute and chronic conditions. She currently practices at GVHP's Urgent Care Center.

Physician Assistant: John Kearney, P.A.-C.
  • Certified, National Commission on Certification of Physician Assistants.
  • Mr. Kearney received his Bachelors degree from Ohio University, followed by a Physician Assistant degree from Western University. He also received a degree in M.S. Health Service Administration from Central University.
  • Mr. Kearney served in the Air Force from 1964-1972 as a medic. He currently practices at GVHP's Urgent Care Center.
